LT Provider Solutions ("LTPS," "we," "our," or "us") respects the privacy of every visitor, client, and prospective partner who engages with our website or advisory services. This Privacy Policy explains what information we collect, how it is used, and the choices available to you.

We collect only the information necessary to respond to inquiries and to deliver our advisory services. This may include your name, organization, email address, phone number, and any details you choose to share through our contact form or direct correspondence. We do not sell, rent, or trade personal information.

Information you provide is used to respond to your inquiry, coordinate engagements, deliver requested materials, and improve the quality of our communications. We may use industry-standard analytics to understand site performance in aggregate; these tools do not identify you personally.

We retain information only as long as needed to serve the purpose for which it was collected, after which it is securely deleted. You may request access, correction, or deletion of your personal information at any time by contacting us directly at hello@ltprovidersolutions.com.

We do not sell, rent, lease, trade, or otherwise monetize your personal information or payment-related data to any third party, including advertisers, data brokers, or marketing networks. Payment information for digital resource purchases is processed directly by Stripe, Inc. under Stripe's own privacy and security standards (PCI-DSS Level 1). LTPS never receives, views, or stores your full payment card number, CVV, or bank credentials. Order metadata (name, email, item purchased, amount) is retained solely for tax, accounting, customer support, and fraud prevention purposes, and is never shared except as required by law or to perform the transaction.

This site is not directed to children under the age of thirteen, and we do not knowingly collect personal information from children. If you believe a child has provided information to us, please contact us so that we may delete it.

By using this website or engaging LTPS, you consent to the practices described in this Policy. We may update this Policy from time to time; the effective date will always reflect the most recent revision.

Terms of Use, Copyright, and Permitted Distribution

These Terms of Use apply to the entire LT Provider Solutions website and all digital resources, templates, checklists, guides, and downloadable materials published by LT Provider Solutions, LLC (collectively referred to herein as the "Site" and the "Resources"). The Physician Onboarding Field Guides and all related digital onboarding products, including their text, structure, checklists, photography, marks, and design, are the copyrighted intellectual property of LT Provider Solutions, LLC. © 2026 LT Provider Solutions, LLC. COPYRIGHT AND INTELLECTUAL PROPERTY NOTICE. All content published on this website and within the field guides is protected by United States and international copyright laws. The compilation, organization, and presentation of all materials are the exclusive property of LT Provider Solutions, LLC. The LT Provider Solutions name, logo, and all related names, logos, product and service names, designs, and slogans are trademarks of LT Provider Solutions, LLC or its affiliates. You must not use such marks without the prior written permission of LT Provider Solutions, LLC. PERMITTED USE. General access to browse and view the Site is permitted for personal and organizational reference. Access to purchased digital onboarding guides and related Resources is granted to a single named recipient organization under a limited, non-exclusive, non-transferable, revocable license for internal physician onboarding use only. Authorized personnel may print and circulate the documents within the licensed organization in unaltered form, retaining all copyright notices, branding, and attributions. Each download is licensed to one organization. If you represent multiple entities, a separate license is required for each. Sharing credentials or download links across unrelated organizations is prohibited.

PROHIBITED USE. Without the prior written consent of LT Provider Solutions, LLC, no person or entity may reproduce, republish, repost, mirror, rehost, upload, distribute, sell, sublicense, share on any public or third-party website, intranet outside the licensed organization, file-sharing service, learning management platform, social media platform, or generative artificial intelligence training corpus, nor create derivative works, translations, or adaptations of the Site or the Resources in whole or in part. Removal or alteration of copyright notices, watermarks, or branding is strictly prohibited. You may not decompile, reverse engineer, disassemble, or otherwise attempt to derive the source structure of the PDF documents.

ARTIFICIAL INTELLIGENCE AND MACHINE LEARNING RESTRICTIONS. The contents of the Site and all digital onboarding guides may not be used to train, fine-tune, test, or improve any artificial intelligence, machine learning, natural language processing, or large language model systems, whether commercial or open-source. This includes but is not limited to scraping, crawling, data mining, or systematic extraction of text, structure, or checklists for ingestion into any AI training dataset.

AI TRANSPARENCY STATEMENT. LT Provider Solutions may use artificial intelligence and machine learning tools internally to assist with research, content organization, drafting, operational analysis, and workflow optimization. Any outputs generated with AI assistance are reviewed, edited, and validated by qualified human professionals before publication or client delivery. AI tools are used as productivity aids, not as substitutes for professional judgment. LTPS remains fully responsible for the accuracy, quality, and suitability of all final deliverables and published materials.

DIGITAL MILLENNIUM COPYRIGHT ACT (DMCA) NOTICE. If you believe that material available on this website or hosted elsewhere infringes your copyright, or if you wish to report unauthorized redistribution of the Site or any digital onboarding guides, please send a written notice to hello@ltprovidersolutions.com containing: (i) a physical or electronic signature of the copyright owner or a person authorized to act on their behalf; (ii) identification of the copyrighted work claimed to have been infringed; (iii) identification of the material that is claimed to be infringing and information reasonably sufficient to permit us to locate the material; (iv) your contact information; (v) a statement that you have a good faith belief that use of the material in the manner complained of is not authorized; and (vi) a statement that the information in the notification is accurate, and under penalty of perjury, that you are authorized to act on behalf of the copyright owner. Unauthorized reproduction or distribution constitutes copyright infringement under 17 U.S.C. § 501 and applicable international treaties, and may result in statutory damages of up to $150,000 per work infringed, injunctive relief, recovery of attorneys' fees and costs, and criminal penalties. LT Provider Solutions employs document fingerprinting, metadata watermarking, and periodic web monitoring to identify infringing copies. We reserve the right to pursue all available legal remedies against individuals and entities who violate these terms.

LIMITATION OF LIABILITY. To the fullest extent permitted by applicable law, LT Provider Solutions, LLC and its officers, directors, employees, agents, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including without limitation, loss of profits, data, use, goodwill, or other intangible losses, resulting from your access to or use of or inability to access or use the Site, the Resources, or any materials available through LT Provider Solutions.

INDEMNIFICATION. You agree to defend, indemnify, and hold harmless LT Provider Solutions, LLC and its licensees and licensors, and their employees, contractors, agents, officers, and directors, from and against any and all claims, damages, obligations, losses, liabilities, costs or debt, and expenses (including but not limited to attorney's fees) arising from your violation of these Terms of Use, your use of the materials, or your violation of any rights of a third party.

GOVERNING LAW. These Terms of Use and any dispute or claim arising out of or in connection with them shall be governed by and construed in accordance with the laws of the State of Florida, without giving effect to any choice or conflict of law provision or rule.

DISPUTE RESOLUTION AND ARBITRATION. Any dispute, controversy, or claim arising out of or relating to these Terms of Use, the Site, or the Resources shall first be addressed through good-faith negotiation between the parties. If the dispute cannot be resolved through direct negotiation within sixty (60) days, either party may submit the dispute to binding arbitration administered by the American Arbitration Association (AAA) under its Commercial Arbitration Rules then in effect. The arbitration shall be conducted in the English language in the State of Florida, before a single arbitrator. The arbitrator's decision shall be final and binding, and judgment on the award rendered by the arbitrator may be entered in any court having jurisdiction. Each party shall bear its own costs and attorneys' fees, except that the prevailing party may recover reasonable attorneys' fees and costs from the non-prevailing party. Notwithstanding the foregoing, either party may seek injunctive or other equitable relief in a court of competent jurisdiction to prevent irreparable harm pending arbitration.

DISCLAIMER. The Site and all digital onboarding guides are offered for informational purposes and do not constitute legal, regulatory, or accreditation counsel. The licensed organization remains solely responsible for compliance with applicable laws, payer rules, and accreditation standards. LT Provider Solutions, LLC makes no representations or warranties of any kind, express or implied, about the completeness, accuracy, reliability, suitability, or availability of the information contained on the Site or within the Resources.

TERMINATION. LT Provider Solutions, LLC may terminate or suspend your access to the Site and your license to the digital onboarding guides immediately, without prior notice or liability, for any reason whatsoever, including without limitation if you breach the Terms of Use. Upon termination, your right to use the materials will immediately cease.

PERMISSIONS AND TAKEDOWN REQUESTS. Reprint permissions, licensing inquiries, tailored editions, and reports of unauthorized hosting may be directed to hello@ltprovidersolutions.com. We aim to respond to all inquiries within two business days.

Terms of Purchase

These Terms of Purchase govern your purchase of digital resources, templates, checklists, guides, and related downloadable materials (collectively, the "Resources") from LT Provider Solutions, LLC through ltprovidersolutions.com. By completing a purchase you confirm that you are at least eighteen (18) years of age, that the information you provide is accurate, and that you accept these terms in full.

Pricing & Payment. All prices are listed in United States Dollars (USD) and are exclusive of any applicable taxes, which will be calculated at checkout where required by law. Payments are processed securely through Stripe, Inc., a PCI-DSS Level 1 certified processor. LTPS does not receive, view, or store your full payment card number, CVV, or banking credentials. We reserve the right to refuse, cancel, or limit any order for reasons including suspected fraud, pricing errors, or unauthorized use.

Order Confirmation & Delivery. After successful payment, you will receive an on-screen confirmation and a link to download your Resource(s), along with a receipt sent to the email address provided at checkout. Download links are time-limited (currently valid for seven days). You are responsible for saving the downloaded files to your own device. If a download link expires or fails for technical reasons, contact us at hello@ltprovidersolutions.com and we will make a reasonable effort to re-issue your download.

License & Permitted Use. Subject to payment in full, LTPS grants you a non-exclusive, non-transferable, non-sublicensable, revocable license to use the Resources solely for your internal business or personal professional use within the purchasing individual's organization. You may not resell, redistribute, sublicense, post publicly, repackage, or use the Resources to develop a competing product, nor remove any copyright or proprietary notices. All Resources remain the exclusive intellectual property of LT Provider Solutions, LLC.

Refund Policy. Due to the immediate digital nature of the Resources, all sales are final and non-refundable once the download has been delivered or accessed. By completing your purchase you expressly acknowledge that you are receiving digital goods immediately upon payment and waive any right of withdrawal to the maximum extent permitted by applicable law. If you were charged in error, received the wrong file, or experienced a delivery failure, contact us within seven (7) days at hello@ltprovidersolutions.com and we will review the request in good faith.

Chargebacks & Disputes. If you have a concern about a purchase, please contact us at hello@ltprovidersolutions.com before initiating a chargeback with your card issuer. Fraudulent or unwarranted chargebacks may result in suspension of access to future purchases.

Governing Law & Dispute Resolution. These Terms of Purchase are governed by the laws of the State of Florida, without regard to its conflict-of-laws principles. Any dispute arising out of or relating to your purchase or use of the Resources shall first be addressed through good-faith negotiation.
